Job Summary
We are seeking a compassionate and dedicated Direct Support Professional based in the Malvern/Exton area to join our team. Our ideal candidate is self-driven, motivated, hard-working and committed to creating a healthy environment that meets patient needs. Someone who operates with a big heart, not only providing personalized and attentive care but building genuine relationships with those you care for.

Strong verbal and written communication skills are needed to succeed in this role.

Prior Home Care experience is a plus.

You must have the ability to work independently, or as part of a team, and be willing to work flexible hours. These flexible hours include evenings, weekends, and holidays..

Responsibilities

Assisting with daily living activities, such as bathing, dressing, grooming, and eating.
Assisting with medication administration, as directed by a healthcare professional.
Supporting individuals with activities of daily living, such as transportation, grocery shopping, and meal planning.
Encouraging and promoting independence and self-care skills among the individuals they support.
Assisting individuals in accessing community resources and activities, such as recreational programs and social events.
Providing emotional support and helping individuals manage challenging behaviors.
Maintaining accurate and timely documentation of services provided and progress towards goals.
Working collaboratively with other healthcare professionals, such as nurses and therapists, to ensure individuals receive coordinated care.
